Personally Identifiable School Records
In compliance with Public Law 93-380, Family Education Rights and Privacy Act of 1974, personally identifiable school records cannot be released without written consent of parent, or student (if 18), unless in compliance with a judicial order or subpoena, in which case students and
parents must be notified in advance. Parents and students of majority age have the right to examine the records, to obtain a copy, if desired, and/or challenge the content of the record.
I hereby authorize my child's previous or current school to release my child's school records to Wakefield Country Day School.
Note: Records should include current and inclusive transcript, national, state, and county test scores, health records, disciplinary, and attendance records.
